Privacy Policy
The primary purpose of this privacy policy is to explain how EOLO-AK protects personally identifiable information (PII) that you submit to us. PII is information that can be used on its own or with other information to identify, contact, or locate a single person, or to identify an individual. This policy also explains how we protect any financial information you submit to us.
This policy describes how and why End-of-Life Options Alaska (EOLO-AK) might access, collect, store, use, and/or share or your personal information when you contact us or submit information or donations to us.
The only information we collect through our website is information you voluntarily submit to us if you request to be added to our email list, submit a comment or question, or if you donate to us.
If you request to be added to our email list, we will collect your first and last name: your email address; your city, state, and zip code; and any comments or questions that you submit via the comment text box.
We store this information on a Google Drive and in a Google gmail account. The Google Drive is accessible only to EOLO AK Board Members and selected volunteers. The gmail account is accessible only to a few Board Members. This Google Drive and gmail are managed and protected by Google under its security standards and policies. EOLO-AK requires that any PII stored on computers is protected by passwords known only to the individual computer user.
We use your contact information to send you communications about our activities, suggested action items, or requests for donations. You may opt out of further communication from us by sending us an email at info@eoloptionsalaska.com requesting to be removed from our email list.
If you request removal from our list, we will delete your name and contact information from our database.
We also use your city, state, and zip code to prepare anonymized summary statistics for our board members and certain authorized volunteers about the geographic distribution of people who have expressed interest in our organization and its mission.
If you authorize us to do so, we also may publish your personal story on our website. We would work with you individually to draft the text of any information posted on our website, and we would obtain your written approval for the text, photos, or videos you share with us before we post it on our website.
EOLO-AK operates under a fiscal sponsorship agreement with the Alaska End-of-Life Alliance (AKEOLA) while it seeks independent status as a 501(c)(3) organization. AKEOLA is a nonprofit, tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code (tax ID number 92-0451618). Under this fiscal sponsorship agreement, AKEOLA accepts donations on our behalf, provides donors with receipts for donations over $250, provides EOLO-AK access to these donations subject to oversight by AKEOLA, and prepares financial reports and reports to the IRS for these donations. Any information you submit to us when making a donation will be shared with AKEOLA so that it can fulfill its oversight and reporting requirements under the fiscal sponsorship agreement and IRS laws and regulations. This fiscal sponsorship arrangement provides to our donors the advantage of having their donations be tax-deductible to the extent allowed by law.
We will not release any information you submit to us to any third party, other than AKEOLA and the bank who receives our donation deposits. Our website is hosted by Squarespace, which also collects information about website users. This information is not available in detail to EOLO-AK. However, we are able to view analytics, which are summary statistics about people who access our website. Please consult Squarespace’s privacy policy for more information about the collection, use, storage, and sharing of data it collects when users access our website.
We use Zeffy to process donations made online to EOLO-AK. When you donate to us through our website, we collect the following information that you submit to us via our “Donate” page: your first and last name; your email address; your city, state, and zip code; and the amount of your donation; and whether your donation is from a corporation or organization or if it is made in honor or memory of someone.
Zeffy also collects information about your credit card number, expiration date, and 3-digit security code or your bank account information to process your donation. However, this information is not provided to EOLO-AK by Zeffy. All that we receive from Zeffy is your name, contact information, and donation amount. Please consult Zeffy’s privacy policy for more information about the collection, use, storage, and sharing of data it collects to process your donations.
If you contact us by mail or email, you provide us your contact information (name and email or mailing address).
If you send us a donation by check through the mail, you provide us your name, address, bank information printed on your check, and your donation amount. We will not store information about your bank or bank account numbers. Once we deposit your check, we no longer have access to your bank or bank account information.
If you send us a donation via our website or in a check, we will store your name, address, email address, and donation amount in order to properly document donations, provide receipts to you, and for AKEOLA to prepare financial reports and submissions to the IRS.
We will store your contact information indefinitely, unless you request that we remove you from our contact list. We will store your financial information related to donations as long as is required and necessary for reporting to the IRS and for our internal financial records.
We have implemented appropriate and reasonable technical and organizational security measures designed to protect the security of any personal information we process. However, despite our safeguards and efforts to secure your information, no electronic transmission over the Internet or information storage technology can be guaranteed to be 100% secure, so we cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security and improperly collect, access, steal, or modify your information. Although we will do our best to protect your personal information, transmission of personal information to and from our services is at your own risk. You should only access the services within a secure environment.
